☀️ Summer is coming to an end and open houses are right around the corner. You’re probably frantically trying to get in last minute doctor appointments, sport physicals, and school forms while trying to enjoy your last family vacation for the summer. Here are a couple of tidbits to keep in mind: 

  • You typically have 30 days to get your forms in to the school. Plan ahead and schedule accordingly to avoid feeling lost and unprepared. If you absolutely cannot make it under the 30 day deadline, show them a copy of your confirmed appointment day/time.
  • Most offices (including ours) do not fill out school forms if your child is not up to date with well checks. If you can’t make it for an appointment, the health department will complete the necessary forms for you. 
  • Pediatricians do not complete the dental part on the 3300 form. You’ll need to see a dentist. 
  • School age vaccination timeframes: 4 years old, 11 years old, and 16 years old.
  • Have your school forms and sports forms printed and ready to be completed at time of visit. Fill out your part ahead of time to get things moving faster. 
  • Be respectful and kind. If you cannot make your appointment PLEASE CALL AHEAD TO CANCEL. Everyone is trying to get in and we are slammed! If you’re running late or need to reschedule it allows us to get others in.
